What each ingredient does

Ripe tomatoes
Form the bulk of the sauce, breaking down when boiled to create its body.
Sugar
Adds sweetness and helps thicken the sauce as it boils down.
Vinegar
Adds acidity that gives the sauce its tang and helps preserve it.
Green peppers
Contribute pepperiness and heat that gives the sauce its character.

The step that matters

Boil until thick before adding the vinegar. Tomatoes and sugar must cook down far enough to reduce water and build body; if the sauce is too thin at this point, it will stay watery after the vinegar and straining are added.
