What each ingredient does

Egg yolks
Thicken the dressing into a custard-like texture when gently heated and add richness.
Vinegar
Adds acidity that sharpens the flavour and balances the richness of the yolks.
Butter
Adds fat that makes the dressing smoother and rounder in mouthfeel.
Fresh milk
Dilutes the thickened custard, loosening it into a pourable dressing.

The step that matters

Steam until custard-like. Egg yolks set through gentle, even heat; if the mixture gets too hot it can scramble, turning grainy instead of smooth. Stop once it thickens and remove from heat promptly.
