What each ingredient does

Milk
Forms the liquid base that the bread crumbs swell into and thicken.
Fine bread crumbs
Absorb the milk and thicken the sauce to a smooth body.
Butter
Adds richness and flavor to the sauce, and browns the coarse crumbs for topping.
Coarse bread crumbs
Brown in butter to add a crisp, toasted texture on top of the sauce.

The step that matters

Cooking the milk with the bread crumbs in a double boiler. Gentle indirect heat in a double boiler keeps the milk from scorching while the crumbs soften and thicken it. Direct heat could burn the milk and bread, leaving a grainy or stuck texture.
