What each ingredient does

Salt mackerel
The main fish; soaking draws out its salt so the flesh becomes mild enough to eat.
Vinegar
Adds acidity that sharpens the flavour of the fish as it simmers in the mixture.
Cream or rich milk
Adds fat and richness, softening the salty, sharp taste of the fish.
Butter
Adds fat and a rounded, savoury flavour to the warm cream sauce.

The step that matters

Freshening the mackerel overnight. Salt mackerel is very salty, and the salt only leaves the flesh through long soaking in fresh water. Changing the water keeps the gradient strong, so a shortened soak leaves the fish too salty.
