What each ingredient does

Green tomatoes
Form the bulk of the pickle and, once salted and simmered, turn tender and translucent.
Coarse sea salt
Draws water out of the tomatoes and onions overnight, firming them before they go into the vinegar.
Vinegar
Provides the acidity that preserves the pickle and gives it its sharp flavor.
Brown sugar
Balances the vinegar with sweetness and helps thicken the syrup as it cooks down.
Onions
Add savory flavor and soften into the pickle during the slow cook.

The step that matters

Cook slowly for 1 hour or until the tomatoes are clear. Slow simmering lets the tomato pieces soften and turn translucent without breaking apart, and lets the spices and sugar fully flavor the vinegar. Boiling hard can make the pieces ragged and the syrup bitter.
