What each ingredient does

Bread crumbs
Thickens the milk into a soft, spoonable sauce as they soak up liquid and swell.
Milk
Softens the bread and forms the liquid base that thickens into the sauce.
Cream or melted butter
Adds richness and a smooth, silky finish to the sauce.
Onion
Infuses the sauce with savory flavor while it simmers, then is removed.

The step that matters

Simmering until thick. Starch from the bread crumbs thickens as it simmers and cools. Stirring well after boiling keeps the crumbs from settling and scorching on the bottom of the pan.
