What each ingredient does

Green tomatoes
Form the bulk of the pickle and give its body once cooked.
Coarse sea salt
Draws water out of the vegetables, firming them and helping them stay crisp.
Vinegar
Provides the acidity that preserves the vegetables and gives the pickle its sharp flavor.
Horseradish
Adds pungent heat that mellows during pickling.
Onions
Add sweetness and aroma as they soften in the brine.

The step that matters

Drain off the water after salting overnight. Salt draws liquid out of the vegetables, and that liquid must be fully drained. Leaving it in dilutes the vinegar and weakens the pickle's acidity and texture.

Swap the onion
