What each ingredient does

Lemon juice
Provides the sharp acidity that defines the lemonade and causes the milk to curdle.
Lemon peel
Adds aromatic oils that flavor the liquid during the long standing time.
Syrup
Adds sweetness to balance the acidity of the lemon juice.
Madeira or sherry
Adds aroma and depth of flavor to the drink.
New milk
Its proteins curdle in the acidic mixture and trap particles, so straining leaves the drink clear.

The step that matters

Pouring the boiled milk into the mixture. The hot milk meets the acid and its proteins curdle, trapping cloudy particles. Pouring it in steadily and then straining through a jelly bag lets those curds be removed, which is what makes the lemonade clear.
