What each ingredient does

Tapioca
Swells and softens in the milk, giving the pudding its thick, tender body as it cooks.
Milk
Hydrates the tapioca and forms the creamy liquid that sets as the pudding cools.
Sugar
Adds sweetness to balance the mild, starchy flavor of the tapioca.
Cream
Adds richness and fat, softening the pudding's flavor and mouthfeel when served.

The step that matters

Boil the soaked tapioca until tender. Tapioca pearls must be fully cooked, because the starch only becomes soft and translucent once gelatinized in boiling liquid. Undercooked pearls stay hard and chalky, and the pudding will not set evenly as it cools.
