What each ingredient does

Potatoes
Break down into a thick, starchy body that gives the soup its smooth texture.
Onions
Add savory sweetness and aroma as they soften in the cooking water.
Cream or rich milk
Adds fat for a silky, rounded mouthfeel and mellows the seasoning.
Butter
Adds fat and richness, carrying flavor and smoothing the finished soup.

The step that matters

Pass the mixture through a sieve. The vegetables must be fully tender before sieving, because firm pieces will not pass through and leave the soup grainy instead of smooth.
