What each ingredient does

Egg yolks
Thicken the cream mixture as they set gently over low heat, adding richness.
Isinglass
Gelatinizes the mixture so it sets firmly enough to hold its shape when turned out.
Savoy biscuits
Form the crisp, absorbent lining that holds the set cream in the mold.
Cream
Adds fat and smooth body, giving the set dessert a rich, soft texture.
Whipped cream
Lightens the custard with air, making it softer and less dense.
Sugar
Sweetens the mixture and balances the flavor of the citrus and liqueur.

The step that matters

Stir the mixture over low heat for a few minutes, then strain it.. Egg yolks thicken and can scramble if overheated, so gentle low heat and constant stirring keep the custard smooth. Straining removes any cooked bits or rind.
